All reporting forms for ISDAR and National are on the Iowa website:

  • Death Report for ISDAR Member, and ISDAR Membership Change Form need to be sent to your State Chaplain and State Registrar. Make sure the death of a member is reported to National, so that State and Chapter Officers receive e-membership Deceased Notification from National. This can be sent by email to the Organizing Secretary General, organizing@dar.org.
    Please forward, in addition to your reporting form, the obituary and funeral leaflet to the State Chaplain to be in the State Chaplain's Necrology Book.
  • National Chapter Chaplain's Report Form CHG-1001 should be used to report request for birthday greetings for members 90 or 100 years old. This request must be sent the 15th of the month prior to the member's birthday. As State Chaplain I will send birthday greetings to 90 year plus members, but only upon request by the member's chapter.
  • Please note: all states and chapters in the United States should use the version provided in Officers (USA) the supplementary document: NSDAR Ritual-Oath of Membership and Installation of Officers, NOT the Oath printed in the newly revised NSDAR Ritual and Missal publication.

    The NSDAR Day of Prayer is October 11, 2015. This is the date closest to the founding of Daughters of the American Revolution October 11, 1890. This is also the Celebration of 125 Anniversary of the founding of DAR. It is the 50th Anniversary of the Vietnam War. Chaplain General Ann Salley Crider has posted a prayer for each of these observances. Please consider these observances when planning your programs. You as Chapter Chaplains are to be commended for keeping in touch with your members in your chapter. This means a lot to members that may not be able to attend meetings. This way they feel in touch and a part of the chapter. Thank you for all you do for your members.
