Make sure the death of a member is
reported to National, so that State and Chapter
Officers receive e-membership Deceased
Notification from National. This can be
sent by email to the Organizing Secretary
General, organizing@dar.org.
- Do not cut the report into strips as this
complicates the filing procedure. Verify
that the member’s entire name (first, maiden
or middle, and last) is given and her
address is current and spelled correctly.
- Use the chapter code with state
abbreviations (e.g., 4-009-WV) to
distinguish between the location of the
chapter and member’s address. Quite
often, members do not live where their
chapters are located. Write dates as
follows: month, date, and year (e.g., Sept.
4, 2018). Please forward, in addition
to your reporting form, the obituary and
funeral leaflet to the State Chaplain to be
in the State Chaplain’s Necrology Book.

The Chapter
Chaplain’s Report Form CHG-1003 should be
used to requests for birthday greetings for
members 90 or 100 years old. This request must
be sent by the 10th of the month
prior to the member’s birthday. As State
Chaplain, I will send birthday greetings to 90
year plus members, but only upon request by the
member’s chapter.
MEMBERS' DEATHS
The Chapter is responsible for reporting all
deaths through eMembership. If a National Board
of Management position was held by the deceased
member, include dates of service, and the name
and address of family member so a condolence
note can be sent by the Chaplain General. She
may also be honored at the Memorial Service at
Continental Congress. On the chapter level,
mention any state office held and dates of
service, since this information is not sent to
us by the National Society. We will memorialize
former state officers at our Memorial Service at
State Conference.
BIRTHDAY GREETINGS
Members will have a birthday greetings sent
from the Chaplain General upon notification on
or before the 10th of month preceding
the birth date. This is for 90 and 100 year olds
. You may send as a e-mail request. Please send
the Chaplain General immediate notification of a
recently deceased member whose name has been
sent in. Requests for birthday greetings
for 91-99-year-olds should be sent to the State
Chaplain.
Remember “You, as Chapter Chaplains and Regents,
set the example of loving kindness as you extend
yourselves to contact those in need of comfort;
celebrate in times of joy and welcome new
members."
